New Patients
To register with our practice, you will be asked to complete a GMS1 form and a registration pack. All patients 16 years and over must attend a new patient medical with our Healthcare Assistant. We have an open list and patients moving within our catchment area are free to register.
You are not registered with one doctor and may consult any doctor in the practice. We do, however ask you to see the same doctor for the duration of a particular illness or problem. For your benefit we feel it is important for all members of a household to be registered with the same practice.
Patients requiring a chaperone during any consultation should speak to the doctors/nurses and this can be arranged.
All patients must complete the GMS1 Form

Any patients requesting access to online services must bring in Photo ID with their forms.
